• Click on CCC online form. Click on to Agree box. Then Click on “I Agree & Proceed”.
• Then Fill up the given information. 
• Photo size-(JPG,JPEG,GIF,PNG image with size upto 50 KB).
• Sign Size-(JPG,JPEG,GIF,PNG image with size upto 50 KB).
• Left Hand Thumb Impression-(JPG,JPEG,GIF,PNG image with size upto 50 KB).
• Then Click on Submit button.
• Then Pay the 590 Rs. Fee.
• Take a Print Out.
